All athletes must complete the following in order to participate in Notre Dame athletics. These forms must be returned to the Athletic Office by July 10 for fall sports and September 10 for all other athletes. All freshmen and first time participants interested in playing a sport during the 2010-11 school year, must attend the athletic orientation with a parent/guardian on Thursday August 25, 2011 at 7:00pm. The meeting will be held in the school gymnasium.

Athlete Physical Exams:
St. Elizabeth Sports Medicine will be offering our annual pre-participation physical examinations for the 2010-2011 school year beginning on June 4th and continuing through July 23rd. Athletes will schedule a private physical throughout the summer, primarily on Tuesdays and Thursdays from 1:15 - 3:30. Physicals will be offered at a $25.00 cost. Physicals will be held at our Edgewood and Florence offices. Space is limited for physicals and appointments will be filled on a first come first serve basis.

Athletic Training at NDA:
Commonwealth St Elizabeth Sports Medicine provides athletic training coverage to several high schools in Northern Kentucky. The athletic trainer plays an integral part in the prevention, evaluation, and treatment of various athletic injuries. They also work in close contact with team physicians and physical therapist to ensure the athletes are returned to sports quickly and safely.
