Athletic Information & Forms

All athletes must complete the following in order to participate in Notre Dame athletics.  These forms must be returned to the Athletic Office by July 12 for fall sports and September 12 for all other athletes.  All freshmen and first time participants interested in playing a sport during the 2008-09 school year, must attend the athletic orientation with a parent/guardian on Thursday August 21, 2008 at 7:30pm.  The meeting will be held at the Heights on campus.

Commonwealth St. Elizabeth Sports Medicine will be offering their annual pre-participation physical examinations for the 2008-2009 school year on Tuesday, June 3rd and continuing through July 15th. Athletes will schedule a private physical throughout the summer, primarily on Tuesdays and Thursdays from 1:15 to 3:30. Commonwealth St Elizabeth Sports Medicine provides athletic training coverage to several high schools in Northern Kentucky. The athletic trainer plays an integral part in the prevention, evaluation, and treatment of various athletic injuries.  They also work in close contact with team physicians and physical therapist to ensure the athletes are returned to sports quickly and safely.  If an athlete has an injury that you feel needs attention please contact:
